Requests for Records Owned or Maintained by Roosevelt City
The Roosevelt City Recorder’s Office responds to all requests for records made under the Utah Government Records Access and Management Act (GRAMA). Requests for records may be submitted several ways:
- Complete an online GRAMA Request for Records.
- Print and complete the GRAMA Request form and fax it to 435-722-5000.
- Print and complete the GRAMA Request form and submit via email to swisener@rooseveltcity.com
- Print and complete the GRAMA Request form and submit it via mail or in person to:

Requests for records may be submitted several ways:

- Submit a GRAMA request for records using the Utah Open Records Portal
