GRAMA Records Request

Requests for Records Owned or Maintained by Roosevelt City

The Roosevelt City Recorder’s Office responds to all requests for records made under the Utah Government Records Access and Management Act (GRAMA). Requests for records may be submitted several ways:

  1. Complete an online GRAMA Request for Records.
  2. Print and complete the GRAMA Request form and fax it to 435-722-5000.
  3. Print and complete the GRAMA Request form and submit via email to swisener@rooseveltcity.com
  4. Print and complete the GRAMA Request form and submit it via mail or in person to:

Requests for Roosevelt City Police Department Records 

Requests for records may be submitted several ways:

  1. Print and complete the Records Request form and fax it to 435-722-5000.
  2. Download and complete the Records Request form and submit via email to gcarroll@rooseveltcity.com
  3. Submit a GRAMA request for records using the Utah Open Records Portal 
     *Please note, only GRAMA requests can be submitted through the portal. 
  4. Print and complete the Records Request form and submit it via mail or in person to:
