How a child/family can Apply
Children/Families need to be recommended from DCFS, local faith leaders, or school from which the child/family is receiving assistance. Our goal is to help every family in need, but our funding is limited, so priority will be given to families at or below the federal poverty line, as well as those experiencing undue hardships. If we are unable to serve a family, their names/ information will be provided to the Sub For Santa program.
Children/Families also will need to provide proof of residency (utility company or credit card bill, doctor or hospital bill, blank statement or bank transaction receipt, pre-printed pay stub, dated within 60 days of the application)
See the link below for a printable form. As a reminder, it needs to be signed by an eligible sponsoring agency. Completed forms need to be returned to the Roosevelt Police Department by December 8, 2025.
