Request Police Records

Requests for Police Records 

Victims have the right to obtain Police Records relating to their case. Please specify if you are requesting a record as a victim, or their authorized representative, pursuant to Utah Code 77-37 (Victim's Rights), or if you are making a request for Public Information under GRAMA.
 
 Requests for records may be submitted several ways:

  1. Print and complete the Records Request form and fax it to 435-722-5000.
  2. Download and complete the Records Request form and submit via email to gcarroll@rooseveltcity.com
  3. Submit a GRAMA request for records using the Utah Open Records Portal 
     
     *Please note, only GRAMA requests can be submitted through the portal. 
  4. Print and complete the Records Request form and submit it via mail or in person to:

Utah Open Records Portal 

Utah's Open Records Portal (ORP) is the central site from which a user can submit a public records (GRAMA) request to a governmental entity.
